HOLISTIC THERAPY SERVICE AGREEMENT

THIS SERVICE AGREEMENT

AND

Customer
 

WHEREAS, the Practitioner is a qualified provider of holistic therapy services and the Customer desires to receive such services.

NOW, THEREFORE, in consideration of the mutual promises and covenants contained herein, the parties agree as follows:

1. Services Provided

The Practitioner agrees to provide  holistic therapy services to the Customer:

  • Holistic therapy, e.g., massage therapy, aromatherapy, reflexology, etc.

2. Customer Responsibilities

2.1 The Customer acknowledges that it is their responsibility to:

  • Complete a Health Questionnaire prior to treatment & Disclose any known allergies, medical conditions, or other relevant health information to the Practitioner prior to receiving services.
  • Adhere to any guidelines, recommendations, or instructions provided by the Practitioner during the course of treatment.

2.2 The Practitioner shall not be held liable for any adverse reactions or consequences resulting from the Customer's failure to disclose pertinent health information.

3. Informed Consent

3.1 The Customer confirms that they have been informed about the nature of the holistic therapy services, including potential benefits and risks.

3.2 The Customer hereby gives their informed consent to receive the services provided by the Practitioner.

4. Payment Terms

4.1 The Customer agrees to pay the Practitioner the total fee for the services rendered.

4.2 Payment shall be made in full prior to service, unless otherwise agreed upon in writing.

5. Cancellation Policy

5.1 The Customer must provide at least [24/48] hours' notice for any cancellation or rescheduling of appointments.

5.2 Failure to provide adequate notice may result in a cancellation fee 

6. Limitation of Liability

6.1 The Practitioner shall not be liable for any indirect, incidental, or consequential damages arising from the services provided under this Agreement.

6.2 The Customer agrees to indemnify and hold harmless the Practitioner from any claims, damages, or liabilities arising from their failure to disclose relevant health information.

7. Confidentiality

7.1 Both parties agree to maintain the confidentiality of any personal information exchanged during the course of this Agreement, except as required by law.

8. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of England and Wales.

9. Entire Agreement

This Agreement constitutes the entire understanding between the parties regarding the subject matter hereof and supersedes all prior agreements, understandings, and negotiations, whether written or oral.

10. Amendments

No modification or amendment of this Agreement shall be effective unless in writing and signed by both parties.

11. Severability

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
